A Letter from the President
ASDIN is THE organization that has defined the field of interventional nephrology giving structure and identity to the field for nephrologists and other specialists who perform diagnostic and vascular access procedures.  ASDIN recently updated our mission statement to broaden our focus and expand our reach.

The mission of ASDIN is to promote excellence in dialysis access care to improve outcomes for patients with kidney disease.

ASDIN leadership and our committees are working harder than ever before to achieve the challenging mission we have set for ourselves.  It is an exciting time for ASDIN, even as we lead in the face of challenges associated with regulatory and reimbursement changes.

With the broader focus in mind, ASDIN has redesigned our annual meeting to include a new introductory Ultrasound Workshop, a broader and most extensive Associates program to include all involved in vascular access and a special session in collaboration with the Vascular Access Society of the Americas (VASA).  I encourage everyone to save the date – February 15 – 17, 2019, and join us in Atlanta.

Monnie Wasse, MD, MPH, FASDIN
ASDIN President

Public Policy Update

In July, CMS released the 2019 proposed rules:

Physician fee schedule (pertinent to office based access centers)

There is a modest increase for codes 36901, 36902, 36904 and 36905 with corresponding modest decrease for the stent codes 36903 and 36906.
Deadline for comment 9/10

Ambulatory Surgery Centers (pertinent to ASC structured access centers)

There is a severe ~50% reduction for codes 36902 and 36905 because of a change to site neutral payments causing these codes to be valued at the same rate as the office site of service.
Deadline for comment 9/24

You may see a compare of our codes in the attached schedule. (see attached 4-page PDF).

ASDIN has worked for many years with the Dialysis Vascular Access Coalition to protect appropriate reimbursement for our dialysis access procedures.

We want you to consider being involved in two different efforts coordinated by DVAC:

Grasstops (Physician Leaders)

We are looking for physicians and practices that are willing to make actual personal phone calls and/or personal visits to members of Congress and Senators to raise concerns about the changes that have occurred and are proposed and how that affects our patients and practices.  If you are able to participate, contact Tim Pflederer.   DVAC will provide you with the information on how to contact, who to contact and specific talking points.  It is very critical that we identify as many people as possible that are willing to participate.

Grassroots (more information coming soon)

We will be coordinating an email campaign for everyone (patients included) through the DVAC website.  We want to encourage members of Congress to sign-on to a letter that is being developed by three key Congressional leaders - Costello, Lance and Dingell.  The sign-on letter specifically addresses the ASC fee schedule.  The more emails we can generate to Congress, the better.  The more members of Congress that that do sign-on to the letter, the more impact the letter will have on CMS.  There is not a letter on the Senate side, but we will have the option through the DVAC advocacy site to ask our Senators for support of the issue.

Coding Update

The code for fibrin sheath management has been made a column 2 code, with a 1 modifier.  You should be using 36596 with 36581. It can be used with the catheter exchange code as in the past but will require an XU modifier.

We believe many physicians have been attaching a 2 modifier to 36595. Please note that there have been some private insurance company denials stating that this code is now bundled with catheter exchange.

This change will be added to the upcoming ASDIN/RPA Coding Manual revisions for 2019.
