How is the care of the vascular access provided when the patient is dialyzing at home?  What should the patients/care partners learn about the care and cannulating of the vascular access at home?  How do they escalate any suspected access dysfunction to the HHD team and nephrologist for a timely referral for any interventions?  Does the Access Center do anything different when treating an HHD patient, such as any needed changes in cannulation sites or methods or monitoring for reoccurrence of the dysfunction?  Vascular access care is a  critical element for getting and keeping HHD patients/care partners dialyzing at home.
