I don't think that this code makes much sense as all other codes cover the problems related to the access. I usually use this code if the only problem with the AV fistula is the presence of accessory veins that needs to be obliterated for maturation failure. I do not use this code if the problem is a hematoma or hemorrhage related to the accessory vein.
