The tip of the catheter was at the area of the filling defect and place a new catheter past the filling defect.

Contacted the referring doctor for considering systemic anticoagulation. Wonder if this is the cause for sudden death in patients with tunnel catheters

I would start anticoagulation and get CT chest with contrast to better visualize the defect. I agree with placing the tip beyond the defect. Whether that has a risk to kill the patient it’s probably a non zero risk but how high I don’t know.
